Originating in the foothills southwest of Mount Rainier, the Tilton River flows south and west for about 29 miles, entirely in Lewis County, eventually joining the Cowlitz River at Mayfield Lake near Mossyrock, Washington.
The two main tributaries are the East Fork Tilton River and the North Fork Tilton River.
